Your New York Living Will

This is a Living Will — a written record of how you want to be cared for if you become very sick and can’t speak for yourself. It does not name someone to make decisions for you (that’s a Health Care Proxy — a different document). What this does is tell your family, your doctors, and anyone else helping with your care what you would want.

You don’t need to know anything to start. There are no wrong answers. You can stop and come back. Every question is optional.
